This Book containing forty six folios or double pages (this page included) was this day presented to me by the Reverend John Bain Minister of the Gospel in the Canadian Wesleyan Methodist New Connexion Church at Bolton in the District of Bedford Province of Canada to serve as a Register of the all the Acts of Baptism, Marriage and Burial performed by him and his Successors in Office during the current year One thousand eight hundred and fifty eight, and the same was paraphed by me the undersigned Prothonotary of the Superior Court for Lower Canada District of Bedford pursuant to the Law in such case made and provided.

To all to whom it may concern, This is to certify that the Bearer John Bain Minister of the Gospel in the Canadian Wesleyan Methodist New Connexion Church was this day set apart for the Office of an Elder in said Church in such way and manner as the Laws and Regulations of that Church require.
